Smile By Design is committed to protecting the privacy of your personal and health information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs).

What information we collect:
We may collect personal information including your name, date of birth, contact details, Medicare and health fund details, and clinical health information relating to your dental treatment.

How we collect information:
Information is collected directly from you when you make an appointment, complete forms, attend our practice, or contact us by phone, email or through our website.

Why we collect information and how we use it:
We collect your information to provide dental treatment, manage appointments, process payments, communicate with your health fund or Medicare, and comply with our legal obligations as a registered health practice. We may also use your contact details to send appointment reminders or practice updates. You may opt out of marketing communications at any time by contacting us.

Disclosure of your information:
Your information may be shared with dental laboratories, specialist referrers, health funds, Medicare, or other treating practitioners where relevant to your care. We do not sell or rent your personal information to third parties.

Overseas disclosure:
Some of our software providers may store data on servers located overseas. Where this occurs, we take reasonable steps to ensure your information is protected in accordance with the APPs.

Storage and security:
Your information is stored securely in our practice management system. We take reasonable steps to protect your information from misuse, loss, unauthorised access, modification or disclosure.

Accessing or correcting your information:

Complaints
